This website provides a highlight of your benefit options. Please read the information provided on this website carefully. For full details about our plans, please refer to the summary plan descriptions available on the Oatey intranet or from Human Resources.

Who is Eligible?

You are eligible if you are an active, full-time associate working at least 30 hours/week on average. If you enroll in coverage for yourself, you may also elect to provide coverage for your dependents.

Dependents are defined as:

  • Your Legal Spouse – Refer to the Working Spouse Affidavit section below for additional details regarding medical coverage.
  • Your dependent children (natural, adopted and any other child as defined in the certificate of coverage) under age 26.
Working Spouse Affidavit for Medical Coverage

Spouses are not eligible to participate in Oatey’s medical plan if they have access to coverage through their employer. A spousal affidavit must be completed by each associate enrolling a spouse in Oatey medical coverage.

Working spouses may be enrolled in other coverages.

Tobacco Surcharge and Cessation Assistance

A medical contribution surcharge will apply to tobacco users.

If you are a smoker, we offer a smoking cessation program. We will work with you and your doctor to help you stop smoking. If you complete the program, you can avoid the medical contribution surcharge.

When and How Do I Enroll?

You can enroll in benefits in Workday as a new hire or during the annual open enrollment period. As a new hire, you must enroll in benefits within 30 days of being eligible.

If you miss the enrollment deadline, or wish to make changes, you will need to wait until the next annual open enrollment period. Open enrollment is the one time each year in which you can make changes to your benefits for any reason. You will be required to complete the annual open enrollment process, even if you do not wish to make any changes to your benefits.

When is My Coverage Effective?

If you are a newly hired associate, the effective date for your benefits is the first of the month following your date of hire as long as you enroll within 30 days from your hire date. Changes made during the Open Enrollment period are effective January 1st of the following year.

Changing Coverage During the Plan Year

Outside of open enrollment, you may change your coverage when you experience a big change in your life. A qualified change in status includes marriage, divorce, birth, adoption, placement for adoption, or loss of coverage.

You must report the change to the Human Resources Department within 30 days after the event by completing the action within Workday.
